In applying PDSA, you ask 3 questions:
1. What is our goal (i.e. what are we trying to accomplish)?
2. What change can we make that will result in improvement?
3. How will we know the change is an improvement?
In the preparation stage, you answer the first question: What is our goal? You will define the focus of your PDSA project using the charter and goal setting tools. You will also gather an effective team to test changes using the PDSA model.

PDSA Project Charter Worksheet
The charter is a living document defining the focus of an improvement project. In some cases, management at your agency (i.e. a project sponsor) will come to you with a focus area for your team to improve. In this case, it can be helpful to start a charter document with the sponsor of your project to clarify expectations and boundaries. In other cases, an internal team wants to examine a practice area and jointly decide on the project’s topic and goals.
